Custodial Records Storage Services

Scholl’s Family Practice closed their business in March 2020. PLBrandon Records Storage had custodial records storage management responsibility until January 2021. DocuDriven took over custodial records storage management responsibility in mid-January 2021. DocuDriven is a local records storage/document scanning company and is not a health care provider and requires a small service fee to process patient records requests.

In order to process any medical record requests for previous Scholl’s Family Practice patients, all requests must provide the following two items prior to receiving the requested medical records:

1. A $30 service fee paid to DocuDriven using the following credit card payment link: Pay Here
2. Please fill out and submit the DocuDriven Release of Records Form to one of the following locations:

Please note: DocuDriven will not be able to provide the requested medical records until both items identified above have been received.
